Making a referral

Making a referral to us is easy. Simply click the link and follow the instructions to complete our online referral form. Please make sure you fill out every section with clear and concise information, as we may ask for further details if we feel its needed. Also make sure you have attached your clients supporting documents (valid identification, a proof of benefits letter no more than three months old, and a proof of national insurance). Also attach any other documents that may be relevant to your client’s support.

Once a referral has been made, we will do our best to invite them in for a face to face assessment at the nearest date if the client meets our eligibility criteria.

If you have any questions or would like an update on the progress of a referral, call our assessment team at our office.

Please note that at times we are dealing with a high volume of referrals which may cause slight delays in the process.

Please also note that we no longer are accepting self-referrals.

Once a referral has been processed, our assessment team may invite the client in for a face-to-face assessment, visit the client for an assessment.

After an assessment, the client’s case is brought to the allocations team of who determines whether the service is suitable for the client and if we have a place to offer them at the time.

If you have any questions or would like to discuss an ongoing referral, just give us a call at the office and a member of the assessment team will be happy to assist.

We do our best to house a client as soon as possible, however, it is not always possible to give a timeframe as it is often dependent on our current occupancy. We also take care in allocating each individual to ensure we are placing them in the best environment to progress.

Harlon Housing CIC Referral Form

Harlon Housing CIC provides supported accommodation to people with care support and supervision needs.  Dependent on individual circumstances, clients can be supported in supervisory community based shared accommodation.

Guidance and information:

  • Please give clear and concise information about the client
  • Please attach relevant ID & most recent proof of benefits
  • Please do not leave any answers blank

Be advised that in order to be considered for a place in our service, clients must fit the following criteria:

  • Be currently in receipt of benefits
  • Engaging or determined to engage with a service which addresses their support needs (eg. Local drug or alcohol services, counselling, &or other alternative services)
  • Homeless or pending homelessness
  • Has valid ID
